You can deactivate Show 4 thumbnails in folder's thumbnail (instead of 1) in Tools » Options... » Thumbnails » Appearance.

Thanks, but I want to to keep 4 thumbs. The problem is 2 of the thumbs are for pictures I've moved to another folder. Is there any way to update the thumbs for folders?

Have you given View » Rebuild thumbnails a try?

Yes, I have. No luck there.

This seems to be a known issue :-|
(I never ran into the problem myself, because I don't use the cache.)
BTW: Is Auto refresh (Tools » Options... » Browser » Browser) enabled?

There's a (rather inconvenient) workaround:
  1. Go to Tools » Options... » Thumbnails » Cache » Cache maintenance.
  2. Select the folder(s) for which the thumbnail(s) need(s) to be updated and click on Remove.

I had to manually remove the appropriate folders from the cache database, which also removed thumbs for any images in the same directory as the folders meaning I had to go back to those folders and re-create the image thumbs. A bit of a pain but only for already created thumbs on folders, newly created folders with images in won't be a problem!

Back to the original post suggestion: is it possible to customize the folder by changing its thumbnail with another picture? The popular viewer QuickPic for Android has such a feature (called "Change Cover" in the folder's settings).
